As state legislatures look for innovative solutions to the looming nursing shortage, some ideas are getting pushback. California lawmakers proposed a bill in February that would allow community colleges to offer a Bachelor of Science in Nursing degree, but state university leaders oppose, arguing it will do β€œmore harm than good.” Limited available clinical sites can’t accommodate more students, their editorial argues, but they make other suggestions, such as increasing the number of ADN-to-BSN degree programs.

A Continuing Crisis

It’s bad enough that suicide-related visits to the emergency department increased five times over between 2011 and 2020 and that children in the ED must wait longer for mental health care than for other concerns. Now a new study finds that trans/nonbinary youth were 9% less likely to receive psychiatric inpatient admission than cisgender youth, and Black youth were 4% less likely to be admitted than white youth. Nearly half of all youth who spent at least three nights in the ED were never admitted.

Failing to Provide

The federal Individuals with Disabilities Education Act requires children with disabilities to receive a free, appropriate public education like all other children, but that’s not always the reality. A feature from NPR describes how many disabled students don’t receive the accommodations they need to learnβ€”or even physically move around the school. From 2022-2023, more than 19,000 discrimination complaints were lodged at the Department of Education, but federal agencies lack the funding to keep up.

Keeping up with Inclusive Language

Words are key to nursing practice, from explaining patient conditions to consoling families. But language evolves and the pressure to β€œget it right” can feel overwhelming.Β 


Consider the term β€œadvanced maternal age”. This imperfect term denotes a pregnant person 35 or older at delivery and was until recently called β€œgeriatric pregnancy.”


Terminology is often offensive because it wasn’t coined by the people it describes. For example, β€œgeriatric pregnancy” originated in the 1970s when only 7% of physicians were women.Β 


Terms must change, yet there is often pushback. This can stem from fear of getting it wrong or shame from having ever said it.Β 

How can we be inclusive when language constantly evolves?Β 

  1. Mirror language. Use terms preferred by the person you are speaking to.Β 

  2. Get curious. Ask questions and dig into the histories of words.Β 

  3. Accept mistakes. Resist the impulse to give up or attack the corrector. Just as language evolves, people can, too.Β
